Coordination of Supports

When it comes to choosing and coordinating your support, Interactive Community Care works with you to connect you with NDIS providers, set up service agreements, and navigate any complex or unexpected situations along the way. Support coordination helps people with disability understand and manage their NDIS plan independently, offering greater choice and control. We’ll connect you with a support coordinator who can help you with budgeting, life planning, parenting training, resolving crisis situations, developing capacity and resilience and co-ordinating complex supports.

Assistance with Daily Life

Interactive Community Care is approved to provide a range of activities to help you in daily life. From helping you manage personal activities like showering to supporting you with household tasks, such as cleaning, our team of support workers are ready to build your capacity and make your life easier. This includes assisting or supervising personal tasks to develop your skills and help you live as independently as possible. We also work with you to build daily living and life skills to improve your quality of life over the long term and set you up for a better future.

Social and Community Participation

Getting out into the community is important on many levels. It helps reduce isolation, build relationships and create vital social connections for people with disabilities as well as their families. Interactive Community Care improves your access to community groups and events with a range of NDIS-approved support services. These include transport to get you to appointments and activities in the community; support workers to accompany you and help you get around; and assistance to attend camps, vacation and holiday care and other group activities.

Improved Daily Living

Therapeutic supports are key to improving your daily living skills. These include training and development activities to increase your ability to live as autonomously as possible. Delivered through individual or group sessions, we can help develop daily living, communication, social and problem-solving skills. We also assist in building everyday skills, such as catching public transport, managing your NDIS funds, participating in group activities and developing greater confidence and independence in daily life.

Improved Living Arrangements

Interactive Community Care specialises in providing short-term accommodation to meet your individual needs. Our respite services help carers take a break while giving people with disabilities the opportunity to try new things in a new location. In addition to providing various accommodation and respite options, we also guide, prompt and undertake activities to help you find and secure appropriate accommodation. This includes tenancy assistance to help you apply for a rental and advice on how to meet the obligations of your tenancy agreement.

Transport

One of the biggest barriers for people with a disability is access to appropriate and reliable transport. Interactive Community Care can help you access transport options to enable your participation in community, social, economic and daily life activities. From taxis to specialist transport arrangements, our team can help you reclaim your independence and make daily life a lot easier. We can also help you make travel and transport arrangements, plan your trips, access independent travel training and develop independent public transport skills.

Depending on your situation, such as your disability and individual needs, you may receive NDIS funding for support coordination. If you do not have an NDIS plan or do not have support coordination in your NDIS plan, talk to one of our friendly staff to discuss your options.

Interactive Community Care has many well-trained and experienced support coordinators located across New South Wales. To access support, simply follow these three steps:

  1. Contact us using the channel that suits you best – phone, email or website.
  2. Choose a time and place for us to meet so you can tell us what you need.
  3. Wait for us to come up with a plan that’s just right for you.
